SAS operative breeches and clears a hijacked 747 in flight, rescuing a hostage with a final slow-motion shot. On Veteran the sequence must be beaten in roughly one minute — one of the most notoriously difficult missions in the series.

Score-attack replay of the full campaign from Crew Expendable through Mile High Club. Score formula: (mission score + time bonus + remaining-lives bonus) x 2 if no deaths x difficulty multiplier (Recruit 1x / Regular 2x / Hardened 3x / Veteran 4x). Lives carry over between missions.

23 missions, 69 total stars (3 per mission). Stars are earned by difficulty: Normal = 1 star, Hardened = 2 stars, Veteran = 3 stars. Completing on a higher difficulty retroactively counts lower tiers. The Pit is the sole exception — stars are time-based regardless of difficulty.

Alpha — available from the start
Clear all targets in the kill-house as fast as possible. Civilian targets must not be hit. Pop-up and moving threats throughout.
The only time-gated mission. IW developer best time: 0:22.6.
Defend position against multiple enemy waves using only sniper rifles, claymores, and Predator Missiles.
Predator Missiles essential for wave management.
Hunt and eliminate all members of a favela gang. On Veteran: reach 50 kills with no more than 3 civilian casualties.
Cramped favela streets make stray shots a constant risk.
Use silenced sniper rifles to slip past or silently eliminate enemy patrols and reach extraction. On Veteran: complete within 7 minutes.
Time limit rewards efficient stealth routing over direct combat.
Push through waves of invading enemy forces advancing across a bridge to reach the safe zone.
Straightforward assault. Enemy density spikes hard on Veteran.
Bravo — unlocks at 4 stars
One player fights on the ground while the other operates an AC-130 gunship overhead providing close air support.
CO-OP ONLY. AC-130 player uses 105mm, 40mm, and 25mm cannons.
Reach the score threshold using kill-streak multipliers. On Veteran: reach 30,000 points.
Kill-streak bonuses accelerate scoring significantly.
Race through the marketplace and defuse all IEDs before they detonate while enemies oppose you.
Knowing bomb locations from prior attempts is a major advantage.
Speed a snowmobile down the mountain slope dodging trees, rocks, and drops. No combat.
Generally considered the easiest Spec Ops mission. Pure driving challenge.
Second AC-130 co-op mission. One player pinned on the ground while the second operates the gunship against larger, more varied enemy forces than Overwatch.
CO-OP ONLY. Harder than Overwatch.
Charlie — unlocks at 8 stars
Navigate Pripyat in a Ghillie suit, silently eliminating or avoiding enemy patrols while moving to extraction.
Widely cited as one of the hardest missions on Veteran due to detection sensitivity.
Assault through multiple Gulag rooms using slow-motion breaching entries to clear enemies.
Flash-bang grenades are essential for breach entries. Solo Veteran is very hard.
Longer snowmobile course than Race with checkpoints that extend your time limit. Missing checkpoints or crashing costs time.
Learning the optimal racing line is key for Veteran.
Defend a fortified position against 5 escalating waves using sentry guns, thermal scopes, rockets, and claymores. Pre-place defenses before waves begin.
Longest mission at ~11 minutes. Optimal sentry placement is critical. Very hard solo on Veteran.
Fight through the Boneyard, grab enemy intel packages, then extract. Heavy ambush at the extraction point.
Clear the LZ before grabbing intel to avoid the extraction ambush.
Delta — unlocks at 20 stars
Laser-designate targets to guide a Stryker APC through contested streets. Prioritize RPG-armed enemies to keep the vehicle alive.
One of the longer Delta missions (~9 min). Stryker destroyed if too many unlased enemies reach it.
Destroy every enemy vehicle on the bridge using C4, grenade launchers, RPGs, and sentry guns. All vehicles must be eliminated.
Clear infantry cover before engaging vehicles on Veteran.
Sabotage the enemy airbase using only silenced weapons and a supplied heartbeat sensor. Eliminate key targets and plant demolition charges.
Silenced weapons only — picking up unsilenced enemy drops jeopardizes stealth.
Fight through the terminal against multiple waves of riot-shield-equipped enemies. Find ways around or through shield formations using grenades and flanking.
Stun grenades counter riot shields. In co-op: one pins shields, one flanks.
Eliminate all enemies: Ghillie-suited snipers in the woods, Elite Mercenaries in the compound, and multiple Juggernauts.
Hardest pre-Echo mission. Thermal scopes spot hidden Ghillie snipers.
Echo — unlocks at 40 stars
Breech and clear densely packed rooms on the oil rig with both enemies and hostages present. Coordinated breaching prevents enemy reaction.
AoE weapons dangerous near hostages. Flash-bang entries recommended.
Eliminate all 10 Juggernauts using only explosive weapons and a knife. No non-explosive enemy weapons allowed. Widely considered one of the hardest missions.
Popular co-op strategy: lure Juggernauts into a shack and coordinate explosive detonation.
The final and hardest Spec Ops mission. Kill all 15 Juggernauts positioned throughout the oil rig. After the first 5 are down, Juggernauts begin spawning in pairs.
Recommended: flashbangs to stun, claymores at choke points, helipad as final stand. The hardest mission in MW2 Spec Ops on Veteran.

16 base missions, 48 total stars (3 per mission). Most missions: difficulty-based (Regular=1, Hardened=2, Veteran=3). Two time-based exceptions: Stay Sharp and Charges Set. Tier unlocks: Tier 1=0 stars, Tier 2=5, Tier 3=10, Tier 4=15.

Tier 1 — available from the start
Sprint an obstacle course, shoot targets, avoid civilian targets — as fast as possible.
No difficulty selector. Recommended: dual pistols, full sprint.
Board and clear President Vorshevsky's 747, capturing him for Makarov.
Slow-motion breach sequences. Flashbangs essential.
Infiltrate a Russian submarine and prevent a nuclear meltdown before the internal countdown expires.
~5-minute internal timer begins after the thermite phase.
Eliminate enemy strongmen holding embassy staff hostage and rescue all captives.
Sniper-focused. No time limit.
Tier 2 — unlocks at 5 stars
Collect nerve agent samples while wearing a Juggernaut suit, then reach extraction.
Juggernaut suit supplied at start. Good mission for the Serrated Edge achievement.
One player operates remote security turrets; the other advances on foot and must reach extraction within ~4 minutes.
CO-OP ONLY. Split roles: turret operator and field agent.
Collect 5 intel items dropped by enemies, then eliminate the high-value target and reach extraction.
Knife sentry guns to disable without alerting others.
Use stealth to rescue the president's daughter from Makarov's forces before she is moved.
Avoid the patrol helicopter. Stealth approach strongly recommended.
Tier 3 — unlocks at 10 stars
Sprint through an obstacle course, plant explosive charges at all targets, and escape before detonation.
No difficulty selector. Switch between pistols efficiently.
Provide sniper overwatch to support rebel forces, eliminating all designated enemy commanders.
Stealth sniping. Quick-scoping required for harder difficulties.
Destroy enemy communications equipment and data servers with persistent helicopter fire support.
Helicopter support throughout. Co-op recommended but not required.
Locate and defuse all IEDs throughout the area in a Juggernaut suit while one player provides overwatch.
Co-op recommended (split sniper and demolitions roles).
Tier 4 — unlocks at 15 stars
Infiltrate via vehicle and destroy enemy communications servers. Juggernaut encounter near the servers.
Vehicle section followed by building assault.
Retrieve drug contraband from a cartel stronghold using helicopter and ground assault, then extract.
Combined helicopter and ground objectives. One of the harder Tier 4 missions.
Acquire all enemy encryption codes from a multi-floor market complex using the XM25 airburst grenade launcher, then extract from the rooftop.
XM25 airburst launcher is the supplied main weapon.
One player operates an AC-130 gunship while the other moves on the ground to destroy all anti-air emplacements within a time limit.
CO-OP ONLY. Split roles: AC-130 gunner and ground operator.
Survival Mode
Endless-wave co-op mode (1-2 players) on multiplayer maps. Waves escalate in enemy type and count. Cash earned per kill and via end-of-wave bonuses is spent at three Armory stations (Weapons, Equipment, Air Support) between waves.
Wave escalation: Waves 1-3: basic infantry with pistols. Wave 4+: SMGs. Mid-waves: gas-mask soldiers, claymore specialists. Bosses: helicopters (wave 6 on), Juggernauts (wave 9 on), Riot Shield Juggernauts, suicide bombers, attack dogs.
Chaos Mode: Non-stop spawning variant with no wave breaks. Available on 4 maps: Resistance, Village, Underground, Dome.

1-3 stars per mission. Stars unlock Kit tier progression (Assault, Medic, Engineer kits) and cosmetic rewards. Time-based: completion = 1 star, under time threshold = 2 or 3 stars.
All missions are 2-player. Raids require exactly 3 players and a Raid Assignment item (earned via Spec Ops, Multiplayer, or Warzone 2.0).
DMZ is a separate PvPvE extraction mode — not a Spec Ops variant.

Call of Duty: Modern Warfare III 2023 · Sledgehammer Games / Treyarch
No traditional Spec Ops mode. Replaced entirely by Modern Warfare Zombies (MWZ) — an open-world extraction co-op mode developed by Treyarch set in Urzikstan. MWZ uses Act-based narrative missions (Act 1, Act 2, Act 3) completed inside 45-minute extraction matches, with persistent cross-session progress.
